Full-Time and Part-Time: Overall Function: Provide support to clients/residents with mental health needs who have been incarcerated at the county, state, or federal level to integrate into transitional housing to offer a choice between custody and independence. Essential Functions: Provides hands-on instruction to residents on implementing Individual crisis plans and assisting with crisis resolution. Provide support by helping clients live as independently as possible and reach attainable goals to maintain their independence. Aids and support with the resident's Activities of Daily Living and Instrumental Activities of Daily Living as indicated in the resident’s assessment and support plan. On a day-to-day basis, support with meal preparation, household chores, medication management, budgeting and finances, accessing community facilities/activities, shopping, socialization, doctor’s appointments, and providing emotional support.
