Job Description: The SMI Case Manager I plays a vital role in delivering case management, care coordination, and psychiatric services for individuals diagnosed with a Serious Mental Illness (SMI). The SMI Case Manager I supports clients through continuous assessment, treatment planning, and service coordination across both mental and physical health systems. This role also involves managing the court-ordered treatment process, ensuring accurate documentation, and coordinating with clinical teams and legal counsel.
