BASIC PURPOSE: The purpose of the Intake Coordinator position is to serve as the primary access point for social service programs by coordinating intake, screening, and referral processes; supporting service enrollment and scheduling; and ensuring accurate data collection and documentation to support Case Workers, the Lead Case Worker, and program operations, including Medically Tailored Meals (MTM) and Medically Tailored Groceries (MTG).
