What Nurse Navigator contributes to Cardinal Health Nursing is responsible for the delivery of safe and efficient quality nursing care. Examines patients and administers prescribed medications to facilitate healing and increase comfort. Nursing requires training and or licensed to care for patients. The Nurse Navigator is a registered nurse who acts as patient advocates, guiding individuals through the healthcare system by coordinating appointments and educating them about their disease and treatment options. This position provides comprehensive care coordination for a disease-specific group of urology patients. Responsibilities: - Manage the case load of patients with chronic disease under the care of our physicians along with Medical Assistant - Serve as a focal point for the patients, directing them to the appropriate resources compassionately and in a timely manner - Assist patients with understanding the disease process, treatment options and through consultation planning, assist them with optimizing visits with their physician. - Ensure knowledge of resources and treatment options are consistently up to date - Improve healthcare access for patients and promote client knowledg...
