Patient Navigation: Provide individualized assistance to clients to help overcome healthcare system barriers and facilitate timely access to quality screening and diagnostics as well as initiation of treatment for women who are diagnosed with cancer. This includes but is not limited to: Conduct patient and provider call, send referrals to the appropriate facilities, completion of the patient report form (PRF), tracking enrollment for screening, re-screening, diagnostic, and treatment services.
