Pharmacist Specialist - Anticoagulation

Job description

Effectively performs the activities and duties of the Pharmacist positions with responsibilities including distribution, consultation, education, research and supervisory functions. Provides an expert level of guidance on medication use in the specialty area of practice. Serves as preceptor for Pharmacy Education program(s). Leads quality improvement projects in the specialty area of practice. Graduate of an accredited College of Pharmacy (Pharm.D. or B.S.) and completion of one of the following: an accredited PGY 1 pharmacy residency and an accredited PGY2 pharmacy residency in area of specialty OR an accredited PGY1 pharmacy residency and at least two years of relevant pharmacy practice experience in specialty area OR at least 2 years of relevant pharmacy practice experience and at least 2 years of relevant pharmacy practice experience in specialty area.
