Nurse Clinician - Inpatient Little Hero Continuity of Care (80%) - ( 26002016 ) Description University of Iowa Healthcare Department of Nursing is seeking a NICU Little Hero Continuity of Care Nurse Clinician to perform specific tasks to assist members of the Nursing team in providing patient education and discharge planning under the supervision of the Director of Pediatric Care Coordination Division. Responsibilities will include, but are not limited to: - Plans, monitors, and documents patient care across the continuum from inpatient and outpatient services - Educates patient and family about aspects of treatment plan. - Serve as a patient advocate as well as assist families to develop advocacy skills. - Apply expert skills to facilitate assessment of patient needs throughout episode of care. - Complete community referrals upon discharge (AEA, Early Access, school systems, etc.) - Observes for patterns of breakdown in plan of care and facilitates correction of problems. - Collaborate and communicate with health care providers from external agencies to assist the facilitation of a smooth transfer to and from the hospital setting. - Identify performance opportunities and assist wi...
