Social Worker, Care Management Monday-Friday, 8am-4:30pm Full Time Meritus Health is seeking a compassionate and collaborative Social Worker to join our Care Management Team. This role supports patients throughout their acute care episode and ensures smooth transitions post-discharge. Key Responsibilities: Coordinate psycho-social support for patients during hospitalization Develop and implement personalized discharge plans Assess patient needs related to care coordination, resources, and services Collaborate with physicians, nurses, and care coordinators to enhance patient satisfaction Facilitate access to community resources to support recovery and continuity of care Ensure compliance with hospital policies, regulatory standards, and evidence-based practices Qualifications: Education: Bachelor’s degree in Social Work required Experience: Minimum two years in acute care, SNF, physician offices, or community health settings Licensure/Certification: LBSW license in Maryland required Must obtain care/case management certification within five...
