Job Description The Case Manager is accountable for seamless service experience that meets the individual needs of the client and their family, compliance with legislation through proper documentation, and communicating with various levels within the organization in a clear and concise manner. The Case Manager works closely with Intake staff, consults and collaborates with other community service providers, leads by example, and demonstrates professionalism and enthusiasm in all dealings. We are looking for: A full-time contract Case Manager, Hospice & Bereavement (12 Months) Hours of Work: 34-hour work week. Monday to Thursday: 8:30am to 4:30pm and Friday: 8:30am to 3:30pm. This is a hybrid role. Reports to: Manager, Social Work, ADP, Hospice & Bereavement Compensation: Hiring Salary Range: The salary for this role ranges from $65,000 to $76,500 per year, depending on your skills and experience.
