As a Home Health Aide (CNA) you will: - Assists with rising patient care needs - Provides excellent personal care to patients to maximize satisfaction according to Aide Plan of Care - Maintains safe environment in patient’s home - Manages visiting caseload productivity within the appropriate expected time frame - Documents care provided to meet regulatory requirements in Electronic Medical Record (EMR) - Consults with professional staff on patient problems and is involved with long and short-term goals that are consistent with organization’s vision, mission, values and goals - Completes continuing education and competency requirements within designated time frames as specified by Medicare conditions of participation - Provide and/or assist with personal hygiene activities, utilize preventive skin care measures, Perform special procedure...
