Full Time (80 Hours Biweekly) Monday-Friday 8am-4:30pm Every 4th weekend: 2 Holidays/year Sign on Bonus Eligible: General Summary: Provides comprehensive family-oriented nursing services to individuals in the home setting by use of the nursing process in accordance with physician orders, the established plan of care, corporate policy and procedures, and the Nurse Practice Act. Duties and Responsibilities: Essential Functions: - Participates in the development of a written plan of care in accordance with the patient’s diagnosis, patient and family needs, physician’s orders, and the corporate policies and procedures. - Provides selected nursing procedures and treatments to patients as outlined in the plan of care and assigned by the primary nurse. - Notifies physician and other care agencies of changes in patient’s condition and care needs and obtains new orders, as needed. - Assists with coordination of patient care through dialogue with other disciplines involved with the patient and documents same. - Participates in patient care conferences. - Documents observations and intervention in the patient’s clinical record in a timely, legible, and accurate manner. - Functions as a member...
