Description SGA provides a wide range of social services in some of Illinois' most challenged and consistently underserved communities and creates long-term and community-wide change. Counties Served: Champaign, Coles, Douglas, Moultrie, and Shelby MAJOR DUTIES AND RESPONSIBILITIES: - Manage and maintain a maximum caseload of 15 families involved with DCFS. - Display effective inter-personal skills, a dedication to helping youth and families reach their goals, and a strong attention to detail in the coordination of services and care. - Collaborate with families to identify individualized needs and develop a plan of care to meet those needs. - Complete a strength-based assessment of all life domains, identifying both strengths and needs - Facilitate and lead the Family Team in all meetings, ensuring the youth and family voice is heard and respected by all team members. - Confidently lead and engage in effective family team meetings, while being observed by program supervisor or program funders. - Work in collaboration with the Family Team, develop a comprehensive plan of care, assigning responsibilities to each team member to address needs, including both non-traditional services an...
