Department: Social Services: Shift/schedule: Contingent (works as needed): GENERAL SUMMARY: Works under the supervision of the Manager of Social Services. The SW Student Discharge Planner Assistant's primary job function is to provide support to patients and their families, with a focus on discharge planning and making appropriate referrals based on the patient’s age and needs. Performs other duties as assigned. Education: Licensure: Experience: - High School Diploma or successful completion of an equivalent High School Exam Required - Enrolled in Bachelor's degree program required - BLS within 90 days of hire required - Participation in a related college practicum preferred JOB SPECIFIC DUTIES AND PERFORMANCE EXPECTATIONS: The following is a summary of the major job duties of this job.
