The Care Partner is responsible for supporting a defined portfolio of clients within the various Community Care funding subsets, with a focus on Support at Home clients. The role serves as the primary contact point for clients whilst supporting them to maintain their health and wellbeing. Furthermore, the role provides comprehensive oversight of the Client Service Delivery Agreements, ensuring exceptional customer service and facilitating clients’ understanding of available program funding and services.
