Description Southeast Primary Care Partners/Southeast Medical Group (SEMG) is seeking a dedicated and dynamic Quality Specialist to join our team. This pivotal role serves as the crucial link between our clinic's lead physician and the SEMG’s population health team, with a focus on driving and improving quality outcomes for our patients. The ideal candidate will be instrumental in executing key initiatives aimed at enhancing performance on quality measures, actively participating in monthly quality review meetings, and identifying patients for enrollment in SEMG’s clinical programs. Requirements: Key Responsibilities: - Liaison Activities: Act as the primary liaison between the clinic’s lead physician and the SEMG’s population health team to facilitate improved quality outcomes. - Quality Initiatives Execution: Implement and monitor key initiatives within the clinic to enhance performance on quality measures.
