Job Summary: As a Wrap Care Coordinator at CK Family Services, you will facilitate the Wrap-around planning process to the family. The following list identifies skills necessary for effective Wraparound Care Coordination. This list of skills should be conceptualized as a tool kit for Care Coordinators. The list is divided into the four phases of Wraparound, including Phase 1: Engagement and team preparation, Phase 2: Planning, Phase 3: Implementation and Phase 4: Transition.
