Practice Consultant | 10563 | Permanent contract | 37 hours per week | £43,822 to £48,227 per annum | (Grade K) | Kings Lynn Are you a registered Social Worker with experience of hospital discharge and integrated working? Are you motivated by improving flow while also embracing new, strengths-based ways of working that support people to live well in their communities? We are excited to offer a permanent Practice Consultant Social Worker role to work alongside our existing Practice Consultant and Team manager, providing professional leadership and oversight of social work practice within community hospital, health led recovery and reablement settings across West Norfolk and further afield. This role sits at the heart of our developing approach, supporting safe and timely discharges, system flow, and the continued embedding of Community Led Support (CLS) across practice. You will work closely with community hospitals, health partners and social care teams to ensure the right support is in place for people at the right time.
