Primary Duties: Direct Practice Support: - Implement products and execute ACO initiatives with direction from Market Leadership. - Accountable for implementing and supporting wellness visits, transitions of care, active management and ensuring practices meet goals set forth in mutually developed Practice Transformation Plans.
