Job description

Experienced and highly trained pharmacist who works in a specialized area performing high level functions (e.g., Oncology/Infusion Center, Pediatrics/Neonates, Decentralized Roles e.g., Unit Based Emergency Department Pharmacist) - Works in 1 or more specialty areas greater than 50% of the time - Specialty areas include but not limited to: Oncology/Infusion Center, Pediatrics/Neonates, Decentralized Roles (e.g., unit-based ED pharmacist) - Proactively contributes to the decision-making process and the development of patient-centered care plans - Deliver satisfactory and valued services and finding mechanisms to reduce medication costs - Participate in quality, safety and other project work related to the specialized area - Assists in the development of policy and procedures Unit Based: Job Function: 01. Profiling / Transcribing: Reviews patient profiles to assure appropriate therapy based on patient specific information that includes at least the following: age, gender, diagnosis, co-morbidities, relevant laboratory values, allergies, height, weight and BSA, pregnancy and lactation status.
