SUMMARY: The LPN MDS role is responsible for various tasks related to resident assessments and care planning including collecting resident data, collaborating with interdisciplinary teams, and contributing to care planning processes to enhance the quality of resident care. ESSENTIAL DUTIES AND RESPONSIBILITIES: - Conducts required initial, quarterly, annual, and significant change assessments based on Resident Assessment Instruction (RAI) guidelines to ensure compliance with state and federal standards. - Collects, analyzes, and documents data about each resident’s health condition and medical history. - Develops, revises, and reviews person-centered care plans as necessary. - Communicates frequently with residents and their representatives and coordinates care plan meetings. - Collaborates and effectively communicates with nursing staff and facility personnel to coordinate quality care for residents. - Attends Staff/Unit meetings promoting, developing, and fostering a positive working environment. - Functions as an integral part of the Quality Improvement Program and contributes to improving the facility's overall quality of care provided to residents. - All other duties as assign...
