Job Description: Northwestern Medicine has an exciting opportunity for either a Nurse Practitioner or a Physician Assistant to work in our Primary Care at Home department. The APP will travel to patients' homes in the western suburbs area of Chicago, or seeing patients in assisted living facilities.
