Overview: Starting pay of $42.21/hr, Pay will commensurate with experience Valid Ohio driver’s license and proof of auto insurance is required by hospital policy. Must pass a motor vehicle background inspection, insurance eligibility, driving qualifications and training set forth by Nationwide Children’s Hospital and must maintain qualification of insurance guidelines. Job Description Summary: Manages nurse home visitors and the community specialists. Responsible for overseeing the implementation of the Family Connects Model with performance depending on monthly key performance indicators. Technical assistance will be regularly provided by Family Connects International to help achieve these goals. Job Description: Essential Functions: - Immerses into the Family Connects nurse home visiting protocol and performs home visits as needed. - Oversees training of new staff. Establishes and coordinates a scheduling and assignment process for home visits.
