We are working closely with local authority in Liverpool to assist with the appointment of a Reablement Assistant , on a 3-months contract, highly likely to be extended at clients discretion. Rate of Pay : £14.43- £18.84 per hour Summary: The Reablement Assistant provides person-centred care and support to adults to promote independence, wellbeing, and enhanced daily living skills, enabling individuals to remain safely within their own homes and communities. Working as part of an integrated Adult Social Care and Health service, the role supports service users to meet physical, emotional, and social care needs in line with agreed care and support plans. The post holder works collaboratively with allied health professionals, social care teams, and families to minimise risks, maximise independence, and support individuals in achieving positive outcomes through reablement and recovery-focused approaches.
