The Registered Nurse Navigator is a community‑based role that provides health advocacy and support to clients throughout Lehigh and Northampton Counties . The Registered Nurse Navigator h elps individuals access the physical and behavioral health care they need by coordinating appointments, supporting hospital transitions, arranging transportation, and creating personalized plans that promote wellness and collaboration. Responsibilities include, but are not limited to, the following: Provide integrated service coordination for assigned consumers, collaborate with behavioral health navigators, engage internal/external stakeholders, and communicate with all healthcare staff to ensure holistic and necessary care. Assume primary responsibility for developing, writing, implementing, evaluating, and revising treatment goals and plans in collaboration with the consumer and their identified supports.
