Schedule Part Time, Day Shift (40 hours biweekly) General Summary: Processes requests for FBMHS from patients and families and referral sources. Essential Functions: - Interviews patients and family members to obtain information about home environment, family relationships, behavioral health history, and patient's personality traits. - Evaluates data gathered in terms of the behavioral health plan of treatment, available social service programs (FBMHS or other service programs as needed), and completes referrals to FBMHS when patient meets criteria, so that each patient will receive services of maximum benefit to patient. - Assists patients and families to understand, accept, and follow medical recommendations and FBMHS program parameters. - Screens each referral to ensure FBMHS will provide services in a least restrictive environment for the patient and his/her family. - Interprets nature of emotional or behavioral condition and prescribed course of treatment to patients and families. Communicates family issues to other staff members.
