Adventist HealthCare Rehabilitation seeks to hire a Nurse Practitioner Clinical Liaison who will embrace our mission to extend God’s care through the ministry of physical, mental and spiritual healing. As a Nurse Practitioner Clinical Liaison you will: In collaboration with the liaison(s), conduct comprehensive pre-admission evaluations, including chart reviews and face-to-face assessments, to determine eligibility for inpatient rehabilitation. - Serve as the team’s primary point and decision maker for complex patients, clinical triage, and critical decision making (and communicate with referral sources, Rehab physiatry as indicated) - Perform quality assurance audits of the Rehab Referral tracker to ensure the efficient processing of Rehab referrals - Build and maintain strong relationships with hospitals, physicians, discharge planners, and payors. - Promote facility services through educational presentations, tours, and outreach activities. - Provide updates to and facilitate obtaining feedback from referring providers as well as...
