Job Title: Wellness Self Management Specialist - Psychiatry Administration - Rivington - Full Time - Day The purpose of Assertive Community Treatment (ACT) is to deliver comprehensive and effective services to individuals who are diagnosed with severe mental illness and whose needs have not been well met in the traditional outpatient setting. ACT is an evidence-based practice providing psychiatric treatment, case management, and support services delivered by a multi-disciplinary mental health treatment team. The Wellness Self-Management Specialist will work with other ACT treatment team members to engage clients in treatment, identify person centered goals, and empower clients to develop life skills. The Wellness Self-Management Specialist provides services to individuals and in group settings. This is a field-based position and requires community outreach visits with clients in their residences, shelters, etc. to ensure continued engagement treatment.
