Description: JOB SUMMARY: Within the context of a client centered care model and in accordance with established standards of professional practice and the vision and values of the organization, works as part of an interdisciplinary community team to provide clinical and consultative social work services to individuals with chronic health and mental health issues, poverty and victims of violence.
