The Case Manager of Forensic Services Support Program may be eligible for a $1500 new hire incentive bonus if hired and meet all qualifications. Position Summary: The role of the Forensic Services Support Program Case Manager is to provide case management services to incarcerated individuals who experience mental and behavior health concerns. The Case Manager will support the individuals, during and post incarceration, to ensure a stable transition back into the community. This would occur by employing a holistic service/strengths-based approach that addresses conditions within the offender’s life that could contribute to recidivism, joblessness, homelessness, and/or substance abuse relapse. The Forensic Services Support Program Case Manager will be an advocate for individuals and assure there are effective resources to support the individual.
