The BILH Heart Transplant Program is seeking a dynamic nurse to coordinate the care of the cardiac transplant patient throughout the phases of referral, evaluation, and waitlist management. By working collaboratively with the Advanced Heart Failure Care Team, the nurse coordinator will assist in the multidisciplinary management of the heart failure patient from Advanced Therapies evaluations to transplant.
