Responsibilities Clinical Responsibilities: Effectively demonstrates the application of adult and child/adolescent specific competencies - Provides effective clinical assessment of served populations - Formulates evidence-based clinical treatment plan in collaboration with patient - Utilizes assessment data to support a DSM 5 diagnosis - Provides individual, group, and family therapy, utilizing clinical evident therapeutic models and theories according to patient needs as stated in the treatment plan - Effectively collaborates with the Family Medicine treatment team in effort to provide integrated healthcare - Provides patient centered safety planning and safety resources appropriate for populations - Evaluates patient’s treatment for discharge planning - Assures that program standards and objectives are met - Plans and develops creative and relevant therapeutic interventions according to patient’s abilities and needs - Demonstrates the ability to recognize the elements of a crisis state and knows how to deescalate or resolve the situation - Effectively collaborates with community resources to ensure wholistic care and assess for needed referral needs of patient. - Tracks Clinical...
