Job Title : Care Navigator Department : Inpatient Psychiatric: Location : Clifton Springs Hospital: Hours Per Week : 36 (12 hour shifts) Schedule : 7AM or 7PM. Every other weekend rotation. SUMMARY : Assist Unit Leadership in assessing patient’s functioning as well as developing and implementing treatment plans and services as patient’s needs warrant. RESPONSIBILITIES: Provides direct care services to patient on an individual and group basis, to include but not limited to activities of daily living, social, and psychiatric rehabilitation per treatment plan, as directed by Unit Leadership. Participates in the development of a comprehensive treatment plan sharing patient’s needs and wants as communicated by patient directly through patient surveys, collaterals, and treatment team. Documents and communicates patient response to treatment effectively and accurately and in a timely manner according to Program Policies and Procedures. Provides comprehensive care to patients in a professional and caring manner and is able to take appropriate actions and communicate findings to the multidisciplinary team.
