Job Summary The Community Health Worker (CHW) will support outreach and engagement for Drexel HOPE’s mobile medical unit. This role is ideal for someone who enjoys working directly in the community, building relationships with local organizations and residents, and helping connect individuals to health and social services. The CHW will play a key role in strengthening community partnerships, supporting mobile clinic operations, and helping to identify locations where the mobile unit can best serve community members. This position works closely with an interdisciplinary care team to promote harm reduction, improve access to care, and support linkage to ongoing services. This position is grant-funded; employment is contingent upon the continued availability of those funds.
