CLINICAL INTAKE MANAGER- FULL TIME

Description Summary & Objective: The Clinical Intake Case Manager will be responsible for front line clinical assessment with the purpose of increasing census by helping determine eligibility of patients and facilitating their placement in the appropriate level of care. Must work in accordance with all applicable laws, regulations, and Life Care standards Essential Functions: - Must have the necessary skills to perform comprehensive nursing reviews for all incoming referrals and assesses for clinical appropriateness, regulatory compliance, and agency resources to meet the needs of the patient. - Must establish and maintain productive working relationships with providers, referral sources, physicians, health care facilities and the community. - Must be able to discuss cases with physicians and clarify physician orders. - Must exhibit exceptional customer service and a positive behavior towards residents and/or patients, colleagues, physicians and community. - Ability to education physicians, case managers, and insurance companies on facility specific programs encompassing the entire continuum of services. - Communicates with Administrator and Interdisciplinary team on a regular basi...
