Senior Director of Home Health Services - ( 26002669 ) Description In collaboration with UI Health Care hospital administrative leaders, physicians, and community partners, the Senior Director of Home Health Services provides strategic and operational leadership for the comprehensive array of services supporting patients transitioning from hospital to home. This includes oversight of homecare nursing services, home infusion services, durable medical equipment (DME), and home health medical supplies across inpatient, outpatient, and procedural settings throughout the health system and defined retail areas The Director is responsible for setting priorities, evaluating outcomes, and advancing the home health strategy to ensure safe, timely, and coordinated discharges. This leader drives the vision, planning, and continuous improvement of service delivery, patient experience, financial oversight, accreditation adherence and operational performance, working closely with executives, clinical departments, and external providers to enhance continuity of care.
