Job type: Full Time Shift: Days, 8:00am to 4:30pm: Hours: 8: The Intake Coordinator is responsible for maintaining census at or above budget. The Intake Coordinator manages bed availability, accept and admits patients per established clinical capabilities. The Intake Coordinator manages referral systems and responds to admission inquires in a timely manner. Provides administrative support to Administrator, interfaces with Clinical RN and handles referrals from all sources. Primary Duties: - Performs admission of patients to the facility, including; Data Entry in EMR and development of baseline care plans. - Screens admission, using approved criteria, for appropriateness of level of care and bed assignment and confirms room is ready, clean and properly set up for admissions. - Coordinates insurance verification, checks previous skilled days, handles authorizations from insurance companies as needed. - Performs an in-depth, systematic, pre-admission patient assessment and communicates pertinent information to appropriate healthcare providers. - Ability to develop relationships with hospital discharge planners, families, and residents. - Coordinates and/or assists in the patient admis...
