We provide comprehensive primary health and wellbeing services to Aboriginal and Torres Strait Islander peoples throughout Blackwater, Rockhampton, and surrounding regions. About the Role: We are seeking a passionate and community-focused Indigenous Outreach Worker to join our Integrated Team Care / Allied Health Services team based in Emerald. This role works closely with clients, allied health professionals, and community stakeholders to support Aboriginal and Torres Strait Islander people living with chronic health conditions. The successful candidate will help facilitate access to multidisciplinary care, promote health awareness, and strengthen relationships between mainstream primary care and Indigenous health services.
