Behavioral Health Coordinator

Job description

Position Summary: This individual will provide behavioral health support to residents of long term care facilities under the supervision of a licensed professional. They may receive training in a broad range of assessments and interventions that will support treatment specified in individual care plans. Regional travel will be required. Essential Duties and Responsibilities: The following duties and responsibilities generally reflect the expectations of this position but are not intended to be all inclusive, other duties may be assigned: - Conduct assessments that include the behavioral and social needs of the patients - Meet with families and facility staff to coordinate care as recommended by licensed professionals - Assess the patient’s knowledge of their clinical condition and provide education and self-management support - Provide outreach to clients who are having problems with treatment adherence - Assist in the development of the individual goals and behavioral health treatment plan - Coordinate with other agencies and community providers - Maintain timely documentation of work - Serve as an active member of the behavioral health treatment team Education and/or Certificatio...
