Overview: - Department: Inpatient Social Work - Schedule: Full time | Monday-Friday | 8:00-4:30pm CST - Facility: Prime Healthcare St. Mary of Nazareth Hospital - Location: 2233 W Division St, Chicago, Illinois 60622. Free parking Responsibilities: Responsible for the coordination of the various activities of the Case Management Department including utilization management, social services and performance improvement. Assists with development of discharge plans. Assists with case management referrals and authorizations. Maintains and coordinates Medi-Cal (TARS) for presentation to on-site reviewer and files them accordingly or as per facility’s procedure. Assists with Discharge Planning as assigned by interviewing patients/families and gathering data to develop a safe Discharge Plan. Qualifications: EDUCATION, EXPERIENCE, TRAINING: 1. Knowledge of Discharge Planning/ Utilization Management / Case Management terminology and functions, in both managed care and non-managed care environments. 2.
