General case management duties including intake assessment, individual and group counseling, case record management, discharge preparation and aftercare services Schedule: Monday/Tuesday/Wednesday 1:00pm to 9:00pm and Thursday/Friday 9:00am to 5:00pm. Pay Rate: $23.84/hour: Essential functions include but are not limited to: Check daily for newly assigned clients. Complete assessments within 3 business days of intake, Facilitate feedback and creation of ISP within 5 business days of intake. DOC case management sessions are to be held based on the clients assessment scores. DOC clients: Submit monthly progress reports to your supervisor 4 calendar days before due date. Home confinement clients: Collect and review phone records on a monthly basis to ensure compliance with the SOW.
