Disability Services Access Coordinator – Short Breaks Team | 10153 | Secondment Opportunity / Temporary Fixed Term Contract until July 2027 | 37 hours per week | £37,280 to £39,152 per annum | Grade I | County Hall The Short Breaks Team currently supports over 3,500 children with disabilities aged 5–17, and we are seeking a full time Disability Services Access Coordinator to cover a period of maternity leave until July 2027. Families self refer to our service, and we receive over 100 new applications every month, alongside reviewing each child’s Short Break Plan annually. In addition, we provide advice and guidance to Social Workers, practitioners, and a wide range of professionals regarding Short Breaks, eligibility, and commissioned services. As a Disability Services Access Coordinator, you will play a key role in ensuring that families receive a fair, transparent and needs led Short Breaks offer. You will review assessments, identify support needs, and work directly with families to ensure service providers are meeting expectations.
