Key Responsibilities: - Perform initial and ongoing patient assessments, including OASIS evaluation and documentation. - Develop and implement tailored treatment plans in consultation with physicians and clinicians, adjusting as needed. - Collaborate with the interdisciplinary care team to optimize patient outcomes. - Provide education and support to patients and families/caregivers on therapy programs. - Maintain accurate clinical records and report any changes in the patient’s condition to the physician. - Supervise Physical Therapy Assistants in accordance with agency policies and state regulations. - Participate in in-services, case conferences, and meetings to promote coordinated care with healthcare team members. - Identify patient and family/care...
