Job Description: Job Summary: The incumbent of this position is responsible for providing coordination of services for patients receiving Medication Assisted Treatment. Essential Job Functions: Review initial assessment/Initial Discharge Planning Session and identify issues associated with, but not limited to, employment, education, healthcare, social connectedness, and criminal justice. Coordinate outpatient services (Mental Health and/or Substance Use Disorder) as needed. Complete regular communication with referral sources/ primary care regarding progress, transition planning, and pertinent clinical issues and document. Request correspondence from past treatment and primary care. Participate in staffing to assure continuity of care. Make or assist in outside referral of issues not able to be addressed in treatment. Assist in scheduling of treatment and can assist in arranging transportation. Responsible for the continuous development and evolution of treatment plans in an individualized manner.
