Job description

Ohel is seeking a Licensed Mental Health Clinician to provide intensive short-term case management for individuals with Severe Mental Illness that are being discharged following a psychiatric hospitalization. The program supports individuals by assisting them in their transition back into the community. Working on an innovative field-based team, this role will include following up on referrals by contacting hospital social workers, engaging and building rapport with participants while still hospitalized, facilitating optimal discharge plan, ensuring that participants maintain linkages to relevant community providers and resources, offering counseling and skill development. Responsibilities: Qualifications: The ideal candidate is a self-starter who can work independently as well as within a team and has excellent follow up skills. This full-time position is located in Brooklyn. Hybrid schedule offered, with inpatient, home and community visits required. Position includes a competitive benefits package.
