Description Medical Provider - Family Nurse Practitioner (FNP) to provide comprehensive primary care services within our integrated outpatient behavioral health model. This position is based primarily at the Mofford Integrated Clinic and serves patients enrolled across Axiom Care programs through a mix of in-person and telehealth visits. The FNP functions as a core member of an interdisciplinary care team, collaborating closely with behavioral health clinicians, nursing staff, care coordinators, and peer support specialists to deliver coordinated, whole-person care to a complex and underserved patient population.
