About Us: Bidgerdii Community Health Services is an Aboriginal and Torres Strait Islander community controlled primary health care service, operating in the Rockhampton and wider Central Queensland region for more than 30 years. Our services include general practice bulk-billing clinics, social and emotional wellbeing and generalist counselling services, chronic disease management, preventative health screening, children and family health and early childhood education services. About the Role: The Aged Care Support Coordinator supports clients to access appropriate local aged care services by providing advocacy, guidance, and culturally safe support. The role helps clients understand their aged care entitlements, registration, costs, and prepares Elders and families for assessments and plan reviews. The coordinator works closely with assessors and the Aged Care Connector, using culturally safe and trauma-informed practices, with duties tailored to each client’s needs. Duties include.
