Care Guide Partners PACE is a part of Hosparus Health. As a Program of All-Inclusive Care for the Elderly (PACE) provider, we work to empower people 55 and older with the support they need to stay in the homes they know and love as they age. PACE helps seniors who need nursing home-level care maintain their independence by providing all the medical care and support services they need to continue living comfortably and safely at home. Schedule: Monday- Friday: Note: This position will report to the Elizabethtown, KY PACE Center Position Summary: The Registered Dietitian screens and assesses participant nutritional status. Uses pertinent data to plan and implement appropriate nutrition interventions and communicates the information to the Interdisciplinary Team to ensure the nutritional needs of the participants are met. Performs nutritional assessment and provides nutritional education for the participants requiring interventions. Coordinates serving of meals; monitors/coordinates kitchen facilities to ensure standards are met. Coordinates with contracted meal services to ensure meals meet the needs of the PACE participants.
