Through our customer-centric services we aim to achieve greater independence and inclusion for all. THE ROLE: We are seeking a Physiotherapist for a community based aged care role. You will provide Physiotherapy with a focus on restoring, improving, or maintaining your clients’ independent functioning and wellbeing. This may include mobility assessments, home exercise programs, prescription of walking aids, and falls prevention programs. For this role, all living in Perth metro regions will be considered. This is a hybrid role working with clients in their homes, in the office and from home. Duties will include: - Provision of evidence based, and goal directed assessment and interventions to CHSP eligible clients. - Maintain appropriate clinical documentation. - Co-ordinate therapy implementation by Allied Health Assistant - Assist with coordination of referrals, planning, service delivery, and evaluation.
