Job Description Mission: As an independent contractor, or direct hire, the Nurse Practitioner will provide comprehensive primary care to residents as part of the interdisciplinary health care team. Key Responsibilities: Support Clinical team with programing. Qualifications : - Provides comprehensive primary care to residents . - Is knowledgeable of the Resident Bill of Rights and endeavors to respect and promote it. - Conducts person - centered health assessment/physical of residents on admission with specified time frame. - Comply with applicable provincial legislation pertaining to long - term care, Long - Term Care Homes Act 2007 & Regulation 79/10 - Participate in regular focused resident and family care conferences. - Manages the care of residents by providing pharmacological, complementary and/or counseling interventions. - Provides outreach and transitional services to LTCH residents who return to the community. - Collaborates and consults with physicians, director of nursing and personal care nursing staff, interdisciplinary team and external resources regarding resident plan of care. - Provides leadership and involves the interdisciplinary team in quality improvement initi...
