SSYI Outreach Worker/Case-Manager (Full-Time)

Job description

Description The SSYI (Safe and Successful Youth Initiative) Outreach Case Manager works with high-risk young adults to connect them with essential services, with the ultimate goal of reducing violence and criminal activity. The role involves building trusting relationships, conducting assessments, and coordinating a personalized intervention plan. Essential Functions: Outreach & Engagement - Identify, build trust with, and engage proven-risk youth through street outreach and community presence. - Conduct consistent outreach during non-traditional hours (evenings/weekends). - Serve as a mentor, advocate, and credible messenger to SSYI participants. - Conduct risk/needs assessments and assist with intake and enrollment into the SSYI program.
